Duties and Responsibilities
The Registered Nurse Specialist is responsible for providing nursing care coordination services for a caseload of medically complex children who are enrolled in the Childrens Medical Services Network. This position may also include providing nursing support at specialty clinics.
50 Provides care coordination services to patients in an assigned caseload which includes performing and ongoing nursing and psychological assessments, developing and updating child specific care plans, coordinating all aspects of medical care with the family and provider by making appointments, referrals, assisting family with obtaining equipment, supplies, medications, and services, and providing authorization for CMS funding of needed services when necessary. Makes home, hospital, and/or school visits as indicated. Ensures children have access to a medical home and referrals to specialty care as needed.
10 Provides counseling, support, and education to patients and families in relation to the diagnosis and treatment.
10 Coordinates and/or participates in CMS clinics by making patient appointments, completing all pre:clinic paperwork, providing clinical nursing and ensuring recommended treatment plan and follow:up care is provided.
10 Documents all patient care activities in the CMS policies and ensures all necessary documentation is shared with appropriate personnel involved in the childs care.
10 Provides information and participates in any meetings, patient care conferences, CMAT staffings, educational staffings, or provider conferences for the child. Functions as a patient/family advocate and represents the needs of the child in seeking support from community:based agencies.
5 Works closely with social services staff in identifying and meeting the psychosocial needs of the patient and family. Makes referrals to the CMS Social Worker and assists with referrals to community agencies.
